my attempt - just a quick draft:

Intro: Dm - Dm - Dm - Dm

Verse: ||: Dm - C - Bb - F - Eb - Dm :||

Chorus: F - Gm - D - Gm - C - D

Solo over Verse + Chorus (2x)

then Dm - Dm --> Verse, 2x Chorus, End (D)

Isn't there some C as passing chord in the verse as well?

There are three guitars in the song, the main acoustic, the electric solo, and also another acoustic guitar that play some chords here and there...

Well, no, F - Gm - D - Gm - C - D is the right sequence for the chorus.
Many tempo changes in the song, a bit challenging.

I found a little mistake on the last chorus, 4 bars more than expected.

  Well it's a standard structure chart (I'm leading a band) you'd better use when you want to share and speed up the learning curve with your musicians. Not found in any bin, it's only reverse engineering.
